After the license of the Springville campus of the high-profile Provo Canyon School was revoked earlier this month, the Provo Campus has also had its license revoked after receiving 13 citations on July 17.
The Utah Department of Health and Human Services (DHHs) cited the facility for failing to protect clients from harm and abuse, for depriving clients of water and rest, and for using "pain compliance" on a child, among other violations.
