🔹 1. Epley Maneuver: Often considered the gold standard for BPPV. It uses gravity and specific head/body positions to guide displaced inner ear crystals back to where they belong.
🔹 2. Semont Maneuver: Involves moving rapidly from lying on one side to the other. It is another powerful technique to clear the semicircular canals.
🔹 3. Foster Maneuver (Half Somersault): An excellent alternative self-treatment that involves kneeling and tucking the chin to help reset the inner ear.
🔹 4. Brandt-Daroff Exercises: A series of repeated side-to-side movements designed to help the brain habituate to dizziness over time.
🔹 5. Gaze Stabilization: Moving your head side-to-side or up-and-down while keeping your eyes locked on a fixed target. This helps retrain the brain's vestibulo-ocular reflex for better balance.
⚠️ Important Reminder: Vertigo can stem from various underlying causes. Always consult with your doctor or a physical therapist before trying these at home to ensure they are the right fit for your specific condition.

ECT (Electroconvulsive Therapy) is a medical treatment that involves sending a brief electrical impulse to the brain to induce a seizure, typically used to treat severe mental disorders like major depression, treatment-resistant depression, bipolar disorder, and schizophrenia [1][2][3].

*How it works*: ECT changes brain chemistry to help improve mood, thought patterns, and behavior.

*Uses*:
- Severe depression with suicidal thoughts or psychosis
- Treatment-resistant depression
- Bipolar disorder (mania or severe depression)
- Schizophrenia with severe symptoms

*Side effects*:
- Memory loss (short-term, sometimes long-term)
- Confusion
- Headache, muscle pain, nausea
- Increased heart rate, blood pressure changes
- Rarely, prolonged seizures or other complications

*Administered under anesthesia* with muscle relaxants to minimize discomfort and risks.

In Iraq, ECT is available in some psychiatric hospitals, but access and practices may vary.

Dry needling releases trigger points through a combination of mechanical, biochemical, and neurophysiological mechanisms that break the cycle of muscle tension and pain.

Mechanical Release
Physical Disruption: The insertion of a thin, solid needle directly into a "knot" (myofascial trigger point) physically disrupts dysfunctional motor endplates and helps realign knotted muscle fibers.
Local Twitch Response (LTR): The needle often elicits an involuntary spinal reflex where the muscle briskly contracts and then relaxes. This twitch helps "reset" the muscle to its normal resting state and relaxes the actin-myosin edling bonds that keep the muscle band taut

Biochemical Normalization
Flushing Toxic Buildup: Trigger points often exist in an "energy crisis" where sustained contraction limits blood flow, leading to oxygen deprivation (hypoxia) and increased acidity (low pH). The mechanical stimulation of needling promotes vasodilation, increasing blood flow to flush out waste and deliver oxygen and nutrients.
Neurotransmitter Homeostasis: Research shows that successful needling immediately lowers the concentration of pain-sensitizing chemicals like Substance P, calcitonin gene-related peptide (CGRP), and interleukins in the affected tissue. It also helps normalize levels of acetylcholine, the neurotransmitter responsible for over-stimulating muscle contraction.
